Special status plant survey (report cover)

It's a good start but I have three remarks:

1. I would make the "Prepared by/for" texts bold because the fact that the weight is less than the addresses is a bad thing. I would make it just as bold or bolder than the addresses.
2. I'm not sure this geometric font you're using is appropriate. A geometric font may not be bad in this context but that condensed /s and /t doesn't look very nice and such a condensed /t is hard to read. I would actually recommend deleting this typeface from your computer and use something like Gotham or Neutraface instead.
3. I like the bold, condensed typeface you used for the heading in general, but a few things make me think it's not a very professional typeface. The J has a diagonal cut while the whole typeface is so focussed on the vertical structure. Letters like C, O and D are very squared while the S is much rounder and the letter N and particularly W have some big dark spots. You can read about dark spots in the article I posted in my Journal. I'm not sure which typeface you're using but I think DIN would've been a much better choice.
